Digital Therapeutics: The Future of Mental Health

Modern mental healthcare is facing a massive accessibility crisis. However, the rise of Digital Therapeutics (DTx) offers a scalable solution. In Episode 9 of the ReeThink Podcast, host Rudi Adigbli interviews Hannes Klöpper.

As the CEO and co-founder of HelloBetter, Hannes has successfully navigated the world’s most rigorous medical approval processes. Together, they explore why Generative AI is the ultimate tool for therapy, despite current regulatory roadblocks.

Meet the Expert: Hannes Klöpper

Hannes Klöpper is a veteran entrepreneur in the digital space. Before co-founding HelloBetter, he built and sold the education platform Iversity. Today, he leads a team of seven founders at HelloBetter. They have conducted over 30 randomized controlled trials (RCTs) to prove the efficacy of their digital programs. Consequently, HelloBetter is now a leader in the German DiGA market with six approved products.

The DTx Business Model: Curing as a Feature

Investors often misunderstand the unit economics of the Digital Therapeutics market. In traditional SaaS, churn is a failure. In DTx, curing the patient is the primary goal.

  • The Recurring Asset: The sustainable revenue driver is the prescriber, not the patient.

  • Clinical Rigor: Success requires Randomized Controlled Trials to prove symptom reduction.

  • Regulatory Scale: National reimbursement (like DiGA) allows for uncapped growth.

Therefore, the business thrives when doctors consistently refer new patients to a proven, effective digital intervention.

The Conflict: Generative AI vs. Medical Regulation

The most potent tool for mental health is conversation. Generative AI excels at this modality. Nevertheless, a major conflict exists between agile AI and the Medical Device Regulation (MDR).

  • The Predictability Mandate: Regulators require medical devices to perform with 100% predictability.
  • The AI Black Box: Large Language Models (LLMs) are probabilistic and can “hallucinate”.
  • The Result: Current laws struggle to approve a tool that is inherently unpredictable.

Hannes argues that we should not hold machines to superhuman standards. Human therapists are also influenced by their moods and fatigue.

The Hybrid Strategy: "ello" and the Path Forward

To innovate while staying compliant, HelloBetter launched ello. This is an AI-based wellness product that operates outside of medical regulation. This strategy allows the team to:

    • Experiment with machine learning and interactive conversation.

    • Gather proprietary data to refine AI guardrails.

    • Prepare the evidence for a future, regulated AI therapeutic.

    The 10-Year Vision: Integrated Care

The future of mental health lies in Integrated Care Systems. These systems will combine DTx, AI tutors, and human specialists. Furthermore, the goal is to target early intervention in childhood. By treating trauma early, we can prevent full-scale mental health disorders later in life.
